[ INFANTILE Snfantile paralysis, as the name dmplies, is for“the most part & disease of early childhood, about 7§ per gent of the cases occurring In children under flve years of age, though older children and even adults are mot in- frequently attacked. Twenty years ago, though it had long been kpown to physicians as a rare disease, it was of such infrequent occuyrrence that it was quite unknown to the general public, but within that time, and espectally since 1905, it has been accurring In epidemics of constantly inéreasing frequency and extent until the epidemic of 1916, whish swept New_York city, New England and the Middle Atlantic states, resujting in a total of not less than 30,000 cases. Even this number of cases, how- ever, is small compared with the cages of measles, scarlet fever and diphtheria which occur each year, and infantile paralysis is still gne of the In less common Infectious diseases. the last ten years it has cau: than one-fourth as many deaths has measles in this country. Since the epidemic of 1916 the disease has been. on the whole, somewhat less prevalent than in the years from 1910 10,1915, and, though some thousands of cases still occur in the United Stdtes every year, with the probabil- ity that further epidemics “will oc- cur, it may at least be hoped that the 1918 epidemic marked the ecrest of the pandemic wave which had been rising for more than twenty years. Symptoms of Paralysis. What gives the disease a peculiar fmportance, out of all proportion to its frequency of occurrence and mor: tallty, is the paralysis, which results fram inflamm¥tion of the spinal cord and may cripple its victims for years or-even for life. The first symptoms are usually fever, some digestive d turbance, pains in the back and limbs, and sometimes unusual nervous symp- toms. either excessive drowsiness or restlessness; and it is now recognized that many cases have only these symptoms, recovering with no evl- dence of paralysis. In the more severe cases paralysis develops within a few daye; it may be complete or partial, affecting only a few muscles or disabling all the ex- trgmities. As the acute symptoms sybside the primury paralysis tends 10 become less extensive, and a cer- tain proportion of cases progress to complete recovery within a few weeks 5. In other cases this early ent stops short of recovery, a more or less extensive 1 is, which may disappear with ir or more of careful treatment of may be permanent. depending upon the extent of damage to the spinal cord. Cause of Disease. The caus he disease is a minute exner and Noguchi of germ t passes readily through the pores of a dense porce- Jaln fliter, which holds back ordinary becteria. More- over, as it is often difficult to make a disgnoais ip the first two or three days, and as many cases are mild, never developing paralysis, it is im- portant, during an epidemic, to have all cases of fever and digestive di turbance seen promptly physiclan. It js pot to be expected, however, that even the most rigid isolatipn of recognized and suspicious cases and thelr known contacts will effectively prevent the spread of this disease, since there will remain & much larger number of carriers, un- recognized and unrestricted, who will spread the infection. Isolation will, however, yndoubted- K havée some llucl uron limiting the s , and there is reason to suppose that the relatively severe cases, which are easily recognized, are more dangerous than carriers in respeet of giving rise to more severe infections. Severe restrictive measures, such as the closing of schools, churches and theaters and restraining travel are neither prac- ticable nor effective, but parents will do well, dyring an epidemic, to keep their young children at home. This will not afford absolute protection, since the adult members of the household, who eannot pemain in isolation, may themselves become carriers; but the smaller the number of people with whom & child comes inty contact during an epldemic the smaller is the chance of contracting the disea No Means for Immunising. No means have yet been discovered for immunizing children against In- fantile paralysis, nor has any anti- 1 toxin been discovered which is cer- tainly effective in treatment. The most effective treatment in the early stage is the simple care which any intelilgent physiclan will prescribe; but treatment of the residual paraly- sis Tequires an expert orthopedic sur- geon and, above all, patience and peui-tenc- on the part of the parents of the affected child. In cases where improvement appe: to have stop- ped much goed can be accomplished by patiently continuing treatment for ars and sometimes by surgical op- eration.
